To be flush you will definitely will need spacers. The offset of my wheels are 48 too but mines are 8.5 wide.
I have 25mm on the back and 15mm on front but i could be flush on the back with 20mm ones ...

Well i replaced my fuel filter when i replaced the fuel pump and the filter was full of crap + stuck. And the car had 105,xxx so i dont think these filters last for ever.

Did you check the axles boot? They can be broken.
If the car is running good with no leaks i only would worry about filters.
I would change the fuel filter just in case, it could be very dirty and fue...
