i had a similar problem...what did the trick...going back and uninstall everything and putting it all back in again...the clip spring and the thing that is the spacer with the fork was put in wrong......

okay....TRD never had mud flaps...so im guessing those are flaps with vinyl sticker on them or custom made logo on...besides, i had that same problem....don't mount to the very top hole...ill take som...
