well, everyone said it best, straight up and truthful. Just don't say it in an ass way. Comfort her and tell her at the same time. Hard and somewhat contradictory, but possible and the best way. Don't sugar coat, don't let her talk you out of it if you have your mind made up, because if she talks you out of that, its that much harder on her and you over time. Just come over, take her somewhere you two can talk, and just say whats on your mind. Don't think too much into it, and don't plan the conversation or try to predict it, because its never the same as you imagine.
